If you do not see your insurance listed, contact me. I may be able to bill as an out-of-network provider or I may have simply forgot to list your insurance company.

I cannot bill Medicare Health Insurance nor can I provide a superbill for them as they do not recognize massage therapists as eligible providers.
What if you're not in-network with my provider or plan?
I may still be able to bill your insurance as an out-of-network provider or provide a detailed receipt of payment (superbill) that you can submit to your insurance company for reimbursement. It depends on the company and plan. Contact me and I’ll look into it for you.
How do I find out if my plan covers massage therapy?
If you are unsure about your benefits, I recommend calling your insurance provider directly and asking the following questions:

How do I book an appointment for insurance massage?
Simply request an insurance pre-verification form from me and once you’ve filled it out, I’ll verify your benefits with your insurance company and let you know the next steps. Do NOT book an appointment through my website if you want me to bill your insurance company as you will be asked to pay a booking fee which does not apply when your massage is covered by insurance. Once I have verified benefits with your insurance company, you and I will communicate directly about your booking your appointment.
Do I need a referral or prescription?
Some plans require a referral or prescription from a doctor, chiropractor, or physical therapist. I recommend asking your insurance provider or sending me your plan info. I can help you sort out the details.
What do I have to pay out of pocket?
Please note:

You may be responsible for copays, coinsurance, or deductibles, which are determined by your insurance plan - not by me - and are due at the time of service.

You are resonspible for late fees, cancellation fees, and no-show fees which are determined by my policies.

You do not pay any portion of my full rate that is not covered by your insurance company.
Can I use my HSA or FSA card for massage?
Yes, HSA/FSA cards can typically be used for massage therapy services, even if we’re not billing insurance. Many HSA/FSA plans only cover medically necessary massage therapy, so it’s always best to confirm with you insurance company what kinds of services can be charged to HSA/FSA cards.
What Insurance Does NOT Cover
Insurance typically does not cover:

relaxation massage
-
energy work
-
distance work
-
non-medically necessary sessions (depends on the plan)

This is not ALWAYS the case, however. Some plans do allow for relaxation, non-medically necessary massage therapy sessions.
If insurance does not cover a service, it is still available as a private-pay option not billed to insurance. In the case of sessions not billed to insurance, payment is due out of pocket at time of service.
Missed Appointments
Please give 48 hours notice if you need to cancel or reschedule your appointment to avoid being charged a fee that cannot be billed to insurance. The $30 deposit does not apply to appointments billed to insurance, but other fees still apply. Learn more about my fees here.
No Show
If no notice of cancellation is given and the client does not show up to their scheduled appointment, their card on file will be charged $80 that cannot be billed to insurance

I consider a client a no-show when they have not arrived or informed me of a late arrival 15 minutes after their appointment has started. If you are running more than 10 minutes late, please let me know as soon as possible. You may not get the full time, but we can still do good work with the time we have.
Want more time?
If you want a longer session than insurance will cover, you must pay for the additional time at the time of service and this additional time (while noted in your chart) cannot be billed to insurance.
