Insurance Massage
I am currently an in-network provider with select insurance plans for medically necessary massage therapy. If your plan covers massage, I'm happy to bill insurance on your behalf as long as we have all the necessary documentation in place. Questions? Contact me!
Insurances I'm Credentialed With
I am currently in-network for the following providers:
-
a referral or prescription for massage from your doctor, chiropractor, or other qualified provider
-
a copy or photo of your insurance card
-
your ID and contact information
-
completed intake forms before your first session
-
a card on file to cover late cancellation and no-show fees
What You'll Need:
To bill your insurance, I'll need:
-
a referral or prescription for massage from your doctor, chiropractor, or other qualified provider
-
a copy or photo of your insurance card
-
your ID and contact information
-
completed intake forms before your first session
-
a card on file to cover late cancellation and no-show fees
Copays, Deductibles, & Coverage
Please Note:
-
Copays and deductibles are determined by your insurance plan - not by me - and are due at the time of service.
-
I do not charge clients more than the allowed amount set by your plan.
-
You do not pay the uncovered portion of my full rate.
-
You are responsible for any portion (such as copays, coinsurance, or deductibles) your insurance does not cover.
Unsure About Your Benefits?
If you are unsure about your benefits, I recommend calling your insurance provider directly and asking:
-
"Do I have coverage for massage therapy with a licesend massage therapist?"
-
"Do I need a referral or prescription?"
-
"What is my copay or coinsurance amount?"
-
"Have I met my deductible?"
What Insurance Does NOT Cover
Insurance typically does not cover:
-
relaxation massage
-
energy work
-
distance work
-
non-medically necessary sessions

These sessions are still available as private-pay options and are not billed to insurance. In the case of sessions not billed to insurance, payment is due at time of service
Missed Appointments
Please give 48 hours notice if you need to cancel or reschedule to avoid being charged a fee that cannot be billed to insurance.
No Show
If no notice of cancellation is given and the client does not show up to their scheduled appointment, their card on file will be charged $80 that cannot be billed to insurance

I consider a client a no-show when they have not arrived or informed me of a late arrival 15 minutes after their appointment has started. If you are running more than 10 minutes late, please let me know as soon as possible. You may not get the full time, but we can still do good work with the time we have.
Want more time?
If you want a longer session than insurance will cover, you must pay for the additional time at the time of service and this additional time (while noted in your chart) cannot be billed to insurance.
